Nightmare Management

Nightmares are distressing dreams that cause significant emotional upset and disrupt sleep. They are common — most adults experience occasional nightmares — but become a disorder when they occur frequently and impair daytime functioning.

Imagery Rehearsal Therapy

Imagery rehearsal therapy is the most evidence-based treatment for nightmare disorder. You write down a recurring nightmare, identify the most distressing part, and create a rescripted version with a different ending or altered content. You rehearse the new version during the day for about 10 to 20 minutes.

After several weeks of rehearsal, the nightmare content shifts in the new direction. The mechanism is believed to be a form of cognitive restructuring combined with memory reconsolidation. Imagery rehearsal therapy reduces nightmare frequency by 50 to 80 percent in most studies.
